Was ist das Klinisch selbstlimitierende Infektionskrankheit durch verschiedene Picornaviren, welche als Symptome - je nach Erreger einzelne oder gar keine - eine Gastroenteritis zeigt, eine sog. Herpangina (Bläschenbildende und ulzerierende Rachenschleimhautentzündung), einen Befall der Skelettmuskulatur mit Muskelschmerzen und Schwäche, einen Befall der Brustwand- und Herzhüllen mit heftigster Schmerzempfindung (Pleurodynie), schließlich der Ausbildung einer Herzentzündung und einer Meningoenzephalitis (Pseudo-Poliomyelitis). Als Langzeitfolge sehen wir eine Herzmuskelschwäche (dilatatorische Kardiomyopathie), sowie möglicherweise die Entwicklung eines Typ 1 Diabetes mellitus aufgrund einer immunologischen Kreuzreaktion zum diabetogenen Hauptantigen GAD65. Es ist derzeit keine spezifische Therapie verfügbar, weder antiviral noch bzgl. der sekundären Gewebsreaktionen; wenngleich die Patentliteratur viele Optionen verzeichnet, sind doch klinische Studien nicht dokumentiert. Coxsackieviren sind im Tierreich (Kleinnager, Insekten) weit verbreitet und werden im Boden, im Abwasser und im Oberflächenwasser nachgewiesen.
